cost the United States approximately $38.7 billion ("Watch," 2001, p.10) in medical expenses and lost wages for workers hurt on the job. An important part of safety practice in the
workplace is the comprehension of techniques of incident investigation as well as the analysis of incident statistics. While each individual incident deserves attention, a compilation of results and analyses
of statistics could result in preventative measures designed to guard against what is typical. Without such statistical reports it would be hard to judge whether or not an incident is
an unlikely or chance occurrence, or if the type of incident could have been prevented. It should however be noted that while statistics are useful, individual incidents may be investigated
thoroughly regardless of type. All major incidents should be studied as information is potentially valuable to the specific company and others who may use the data to institute preventative measures.
When investigating a major workplace incident, several steps need to be taken to assure that information is recorded properly and conveyed to
